What to Bring
- Insurance card (front and back)
- Photo ID (driver's license or state ID)
- Physician referral or prescription, if applicable
- List of current medications
- Any relevant medical records or imaging results
What to Expect
- Please arrive 15 minutes early to complete paperwork
- Your initial evaluation will take approximately 60 minutes
- Wear comfortable, loose-fitting clothing
- Your therapist will assess your condition and create a personalized treatment plan
- You'll receive guidance on exercises to begin at home
